About Hospice Care
Primary Hospice Care, Inc. is patient-centered and patient-directed. It is one of the best examples in the health care of collaboration by an interdisciplinary team, dedicated to meeting the needs of the patient and family. In consultation with the patient’s own physician, our hospice team prepares an individual plan of care based on the patient and family needs and goals. Direct care is provided by the primary caregiver and is augmented by our hospice team. Hospice care is provided wherever the patient calls home.
We also provide medications related to comfort and the terminal diagnosis, some supplies and durable medical equipment needed. Our services are designed to support the care that the patient’s primary caregiver provides.

- The Patient’s Personal Physician and
- The Hospice Medical Director
- Experienced Nurses
- Social Workers
- Chaplains/Spiritual Care Specialists
- Trained Volunteers
- Home Health Aides
When is it Time?
If you or a loved one has a life-limiting illness and choose not to seek aggressive treatment, it may be time to ask about our services at Primary Hospice Care, Inc. We are ready to walk with you and to prove high-quality care, respect, and compassion at the end of your journey while you choose to maintain the highest possible level of independence within familiar surroundings.
How is Hospice Care Paid For?
Primary Hospice Care, Inc. does not charge any fees to our patients or their families for any of our services. You can set aside your fears of how to afford end of life care. Hospice care is a Medicare / Medicaid along with the most private insurance company certified program, which covers the benefits of hospice care. However, the availability of Medicare / Medicaid or private insurance is not a criterion for admission.
